WELFARE COMMITTEE
This committee has been formed to provide facts and phone numbers regarding insurance, procedures to follow in the event of the death of an employee, spouse or child, problems with prescription drugs, nursing homes/skilled care, etc.

DEATH OF AN EMPLOYEE
1. Determine type of insurance, i.e., Goodyear Veba or Titan Veba (to qualify for Goodyear Veba, you had to be 55 years of age with 25 years of service at the time of the sale in December, 2005. 2. Call the Goodyear or Titan Veba (must provide death certificate and verify address and phone numbers. 3. Call Goodyear Benefit Solution Center regarding the pension, verify address and phone number. Advise that the Goodyear Pension will be stopped until all information is obtained. This could take 4-8 weeks to process. Goodyear will notify Minnesota Life Insurance. 4. Notify Social Security Administration (may need death certificate, marriage license, and both birth certificates) 5. Some employees will have the Steelworkers Pension through Titan. (retired after 2006). 6. Notify 401k plan 7. Advise that the surviving spouse will no longer receive the $50/month from Veba to help defer the payments on Medicare.

1. Contact Veba. 2. Goodyear Benefit Solution Center 3. Optional Life insurance 4. Steelworkers Pension (if applicable) to change beneficiary. 5. Social Security
DEATH OF A CHILD
1. Follow the same procedure.
Advise the spouse/employee to CALL Tony for information on the life insurance. Do not come to the office until the forms are received and need to be completed.
When the employee dies, the surviving spouse will get a new medical card with their name as the employee.
DENTAL COVERAGE
We must notify members that insurance is dropped effective 2/28/2011
Titan’s position is that they do not have to offer COBRA.
Delta Dental has volunteered to offer 2 programs.
Delta Dental also has the Union pay half the cost of the mailing and provide envelopes to mail the forms to the members.
PRESCRIPTION DRUGS
When making calls to Express Scripts, if the individual is having problems, ask to speak with a manager.
INSURANCE QUESTIONS
Advise the employee/spouse to have the Highmark BC BS EOB in hand when calling the office for help. |
